QL_FAIL(..) file and line info

QL_FAIL(..) file and line info

Hello all,

What do I need to do to get QL_FAIL(..) and QL_REQUIRE(..) to report the file and line-number when they throw?
I see from their definitions that the can sometimes report them.

Is it ever possible to get the call stack reported as well?
